Initial Consultation
The physician will come to your house and get to know you and your support people, go over your medical conditions, and make an assessment and recommendations based on what is discussed.

Follow-up Care
The team will do routine or as needed follow up visits on an as needed basis, or you can decide to have full access to the team whenever needed, on a monthly fee basis

Counseling and Resource Support
Our Palliative Care trained Licensed Clinical Social Worker offers support and counseling for a myriad of issues related to chronic serious illness - loss, grief, caregiver burnout, existential and spiritual stress, emotional and psychological issues and much more. Our LCSW also offers support with finding resources for finding home health aides, meal services, transportation and the like

Advanced Care Planning
Our team will lead the completion of relevant documents such as advanced directives, health care proxy forms and/or POLST forms. We will lead family meetings to discuss goals and wishes, always with emotional and spiritual support as top priorities

Monthly Access
Have unlimited montlhy access to teh entire team. This is for when you become sicker, or you need extra support through at particular time.
